If you have one of the conditions listed below, you are eligible for the Nurse Coaching program. When you enroll, you will work one-to-one with an Arkansas-based Nurse Coach at no cost to you.

• Congestive Heart Failure
• Chronic Obstructive Pulmonary Disease
• Diabetes
• Chronic Pain
• High Cholesterol
• High Blood Pressure
• Chronic Kidney Disease

Special Delivery (Maternity Program)
The Special Delivery program was created with you and your baby’s health in mind. By focusing on prenatal care earlier, you’re improving the long-term health of your baby. The Special Delivery program offers many benefits for families who are expecting. The Special Delivery program is available to you and your covered dependents, at no cost to you. You may register up to 20 weeks gestation.
A $250.00 check will be mailed to you once the completion of the program has been verified.

Case Management
If a plan member or dependent has an illness or injury that’s difficult, long-term or costly, they can work with a case manager to make sure they’re getting the care they need. The case manager can help them understand treatment options and their benefit plan. We have nurse case managers who specialize in oncology to help members who have been diagnosed with cancer.

Nurse24
Nurse24 is available 24 hours a day, 7 days a week. When you call, you will speak directly with a nurse who can answer health questions or help you decide if you need to see a doctor or go to the emergency room. If you are seeking advice for emergency care, the co-pay is waived for Premium members who
are advised to seek care at the ER through the toll-free nurse line.

Employee Assistance Program (EAP)
